For Hospitals

Nearly 3,000 general hospitals are issued a Leapfrog Hospital Safety Grade twice per year. Using up to 22 evidence-based measures of patient safety, including CMS Medicare PSI 90 Patient Safety and Adverse Events composite which includes 10 component measures, The Leapfrog Group calculates a numerical score for all eligible hospitals in the U.S. The Safety Grade uses national publicly available data from a variety of sources. However, because of inadequate data, we are unable to assign a grade to certain hospitals, such as critical access hospitals, specialty hospitals, children’s hospitals, outpatient surgery centers, etc.
